New Patient & Existing Patient
Services are provided on a sliding scale, if needed. CHC cannot refill medications until you have been seen by your new provider, and controlled medications are not prescribed at a new patient visit.

We can help you determine if you are eligible and complete the application, including MassHealth and other programs for eligible Massachusetts residents. If you lose your coverage, we’ll help you find other insurance or reapply. Please don’t miss an appointment because of an insurance issue — we are here to help.
